In this vital role, you will provide personalized in-home care to clients across diverse needs, ensuring their safety, comfort, and independence. This position offers an opportunity to work closely with individuals requiring assistance with daily activities, fostering a trusting relationship and promoting well-being in the comfort of their own homes. Your proactive approach and attentive care will help clients maintain their quality of life while supporting their health and safety. Responsibilities Assist clients with daily living activities such as bathing, dressing, grooming, and mobility support. Administer medication reminders and ensure adherence to prescribed health routines. Monitor clients' physical and emotional well-being, reporting any changes or concerns promptly. Provide companionship and engage clients in meaningful activities to promote mental stimulation. Maintain accurate documentation of care provided, including observations and any incidents. Coordinate with family members and healthcare professionals to ensure comprehensive care plans are followed. Adhere to all safety protocols and infection control standards to protect both clients and yourself. Skills Strong interpersonal skills with a compassionate and patient demeanor. Excellent communication abilities to effectively interact with clients, families, and healthcare teams. Ability to follow detailed care plans and instructions accurately. Experience working with diverse populations, including geriatrics and individuals with chronic conditions. Knowledge of medical-surgical procedures, post-acute care practices, and outpatient services is highly desirable. Prior experience in hospital medicine or emergency medicine settings is a plus, especially those involving Level I Trauma Centers or Level II trauma center experience. Familiarity with PICU (Pediatric Intensive Care Unit), OR (Operating Room) procedures, teaching hospital environments, or sub-acute care settings enhances your suitability for this role.
