Job Description

Job Summary We are seeking a compassionate and experienced Lead Caregiver/Supervisor to oversee the daily operations of an Adult Family Home from 7am to 7pm. This role involves supervising caregiving staff, ensuring high-quality patient care, and maintaining a safe, supportive environment for residents with various needs, including dementia, developmental disabilities, and other long-term care requirements. The ideal candidate will have a strong background in senior and home health care, excellent leadership skills, and a commitment to enhancing residents' quality of life through personalized care plans and attentive support. Overtime opportunities are available for those interested in additional hours. Duties Supervise and coordinate caregiving staff to ensure consistent delivery of personalized resident care Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Administer medications and monitor vital signs following HIPAA guidelines and care plans Provide patient observation and monitor behavioral or health changes, especially in residents with dementia or Alzheimer's Prepare nutritious meals in accordance with dietary restrictions and assist with meal planning and preparation Maintain accurate documentation of resident care activities, medication administration, and progress notes Ensure a clean, safe environment by overseeing cleaning routines and adhering to infection control protocols Support residents' emotional well-being through companionship, social activities, and community engagement Collaborate with healthcare professionals to develop and update individualized care plans Assist with mobility using equipment such as Hoyer lifts or resident transfer devices when necessary Ensure compliance with all regulations related to home health, hospice care, assisted living, and disability services Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings Knowledge of Alzheimer's/dementia/memory care, autism spectrum disorders, developmental disabilities, and long-term care practices Certifications such as First Aid, HIPAA compliance preferred; willingness to obtain or renew certifications required Experience with medication administration, vital signs monitoring, patient observation, and ADL assistance Strong leadership skills with the ability to supervise staff effectively while fostering a team-oriented environment Excellent communication skills for working with residents' families, healthcare providers, and team members Ability to perform heavy lifting when necessary for resident transfers or mobility assistance Compassionate attitude with a dedication to providing respectful and dignified resident care Flexibility to work extended hours or overtime as needed to meet resident needs This position offers an opportunity to make a meaningful difference in residents' lives while working in a supportive team environment dedicated to compassionate senior care.
