Special Women (including mothers in recovery and with children) Position Summary The Case Manager at Olivia's House is responsible for providing comprehensive case management services to women in a residential substance use treatment program. This position focuses on helping clients achieve stability, access essential resources, and successfully transition into independent and recovery-oriented living. The Case Manager works closely with clinical staff to support individualized service plans and ensure continuity of care. Key Responsibilities Client Support & Case Management Provide direct case management services to women in treatment Assist clients in developing and achieving individualized service plan goals Monitor client progress and document all services in accordance with agency and ADMH standards Participate in treatment team meetings and staffing Resource Coordination Assist clients with obtaining essential documents, including: State Identification (ID) Birth Certificates Social Security cards Link clients to community resources such as: Housing assistance Employment and job readiness programs Educational and vocational services Healthcare and mental health services Transportation & Advocacy Transport clients to appointments, including: Medical and mental health appointments Court dates Social service agencies Advocate on behalf of clients to ensure access to needed services Life Skills & Reentry Support Provide support with life skills, including: Budgeting Time management Parenting support (as applicable) Assist with discharge planning and transition to independent living Documentation & Compliance Maintain accurate, timely, and complete case notes and documentation Ensure all services are documented according to agency policies and billing requirements Follow confidentiality guidelines and HIPAA regulations Qualifications High School Diploma or Bachelor's Degree in Social Services or related field preferred Five years of experience working with women, substance use disorders, or residential treatment preferred Valid driver's license and ability to transport clients Certification Requirement Must obtain Case Management Certification within 6 months of hire Skills & Competencies Strong communication and interpersonal skills Ability to work with diverse populations, including women with trauma histories Knowledge of community resources Ability to maintain professional boundaries and confidentiality Organizational and documentation skills Work Environment Residential treatment setting serving women and families Full time 9:00 - 5:00 pm.Pay:
