Full Job Description Job Description:
The WellStar / CRC Case Manager provides resources for care, assists the therapeutic team, and encourages healthy choices as your clients transition to outpatient therapy and treatment. This position will be working Monday - Friday, 8:00AM - 5:00PM. What you'll be doing: Provide outpatient case management services to the seriously mentally ill population. Provide direct services to severely mentally ill clients to address needs which are unmet as well as information about the continuum of care. Provide training on money management as it relates to meeting needs of the client to obtain and maintain psychiatric stability in a residential setting. Provide follow-up progress updates as needed for the client to treatment team members. Evaluate clients' residential stability and assist clients who are in need of a more stable environment. Provide Basic Living Skills training when necessary as it relates to clients' immediate needs while preparing client for a less intensive level of care within the community. Assist clients with obtaining medication. Monitor medication compliance, maintain awareness of common side effects, and ensure client is taking medication as directed on a regular basis. Provide traditional case management services as well as collateral services such as transportation to appointments, referral agencies, etc. Maintain high-quality client documentation and records management through the utilization of electronic charting. Monitor the care and services clients receive in the residential setting. Develop a positive working relationship with the Care Director to meet needs of clients. Develop and implement Individual Service Plans, which consist of goals, resources, and interventions. Review client's progress every three months and revise Individual Service Plan as needed. Review SUN-R 6 months after initial assessment and then at intervals of 12 months thereafter. Revise client's goals once met, or progress as they occur. Assist mentally ill clients through crisis situations and/or arranging for the provision of such assistance by other professional or personal caregivers. Perform incidental care for families of the mentally ill and assist those family members in understanding the mental illness. Complete other duties as assigned.
