Job Description

Overview:
The Case Manager supports clients in addressing non-vocational barriers that impact their ability to secure and maintain employment. This role focuses on connecting clients to essential resources—such as housing, healthcare, food, and transportation—while providing individualized, trauma-informed support that promotes stability, independence, and long-term success.
Key Responsibilities:
Assess client needs to be able to sustain employment Connect clients to community resources (housing, healthcare, SNAP, Social Security, etc.) Provide support with life skills such as budgeting, scheduling, and navigating systems Coordinate care with internal staff and external service providers Monitor client progress and follow service plan Maintain accurate and daily case notes Attend client meetings and collaborate with team members to support client success
Qualifications:
High school diploma or GED required One of the following: Associate's degree or higher in social work, behavioral health, or related field Peer/Recovery Support Specialist Certification 1+ year of related experience Strong communication, organization, and problem-solving skills Ability to work with diverse populations in a trauma-informed, culturally sensitive way Basic computer proficiency (Microsoft Office)
Work Environment:
Onsite, client-facing role
Additional Requirements:
Ability to pass background check and drug screening CPR/First Aid certification (or willingness to obtain)
