Case Manager Responsibilities :
Conduct comprehensive assessments of residents' needs, including medical, social, and housing needs. Develop individualized care plans in collaboration with residents, healthcare providers, and other stakeholders. Coordinate access to medical care, mental health services, substance abuse treatment, and other supportive services as needed. Advocate for residents' rights and access to resources, including housing, benefits, and community support services. Monitor residents' progress towards goals and adjust care plans as needed to promote positive outcomes. Provide crisis intervention and support to residents experiencing challenges or setbacks. Maintain accurate and up-to-date documentation of case management activities and resident progress.
Case Manager Requirements:
Previous experience in case management or care coordination, preferably in a healthcare or social services setting Compassionate and empathetic attitude towards individuals from diverse backgrounds, including the underserved population. Knowledge of community resources and social service programs available to individuals in need Strong interpersonal and communication skills, with the ability to build rapport and establish trust with diverse populations Excellent organizational and time management abilities, with a keen attention to detail Ability to work independently and as part of a multidisciplinary team in a fast-paced environment Proficiency in electronic medical records (EMR) and documentation software preferred Valid driver's license and reliable transportation required
