Case Manager-Emergency & Interim Housing Dream Live Hope Foundation Compton, CA 90222 $26 an hour - Full-time $26 an hour - Full-time Organization Dream Live Hope Foundation (DLH) is a minority 501 (c)(3) nonprofit organization that helps fill the housing void for people who are in need of assistance. For more information, please visit https://www.dreamlivehope.org Position We are currently seeking to fill a Case Manager role to service our Emergency and Interim housing participants. The Case Manager is responsible for performing several tasks to achieve the best outcomes for the participants. These duties usually involve working with participants, their family members, close friends, care team and community partnerships. Responsibilities Able to conduct initial and monthly assessment with participants referred to and implement an individualized service plan, in collaboration with program participants, based on the information gathered through the program assessment. Manage CMS and other databases for all Emergency and Interim program participants. Collaborate with participants to maintain goals conducting regular follow up sessions to monitor progress, address changes, and provide ongoing support. Digitize action plans into workflow systems to track progress and milestones effectively. Develop professional relationships with the program participants and maintain a caseload of 20-30 participants. Work in conjunction with Housing Navigators and program partners to achieve the best outcome for program participants. Ensure that participants are linked to health, mental health, and substance use services as needed. Assist participants with obtaining necessary documentation. Utilize a harm reduction, "client-focused" and strengths-based intervention and treatment model. Utilize appropriate services and programs that are a part of Dream Live Hope. Participate in all required team and collaborative agency meetings. Attend all required staff training as well as pursuing continual education in the field of case management. Perform any other duties related to the efficiency and effectiveness of the Case Manager position. Qualifications Minimum of 2-4 years of direct experience in Case Management with a caseload of 20+. Experience with CMS database. Compassionate and able to relate to different clients with various needs. Motivational Interviewing to encourage clients to follow their care plans. Strong emotional intelligence and conflict resolution skills. Critical thinking and problem solving to determine the best care plan for each client after assessing. clients, analyzing notes from the care team. Flexibility to change care plans if they are not getting the best results. Organizational skills to manage several different cases at once. Detail oriented and administratively skilled. Fluent Spanish/English speaking Preferred Qualifications Bachelor's degree preferred in social work, psychology, counseling, or related field; relevant experience may be considered. Experience in the mental health and/or substance abuse field. Proven experience in case management, resource advocacy, or social services. System impacted and/or lived experience preferred.
