This is an active, community-based position—not a traditional desk-centered case management role. You will work directly with a small caseload of adult members, helping them strengthen the skills, confidence, and connections needed to live as independently as possible. Your responsibilities will include:

Developing positive, respectful, and professional relationships with the members you support Teaching independent living skills, including household maintenance, organization, budgeting, and time management Assisting with meal planning, grocery shopping, and food preparation Supporting personal hygiene, healthy routines, and other daily living needs Monitoring and supporting medication routines when required Helping members strengthen social skills and build meaningful community connections Promoting personal wellness, home safety, self-advocacy, and informed decision-making Connecting members with community resources and opportunities Maintaining accurate documentation and communicating important information to the appropriate team members Collaborating with coworkers, families, service coordinators, and other members of each person's support team Following through on assigned responsibilities and responding proactively when members need assistance Who Will Thrive in This Role You may be an excellent fit if you: Approach your work with patience, optimism, and respect Are dependable and understand that consistency matters to the people we serve Enjoy helping others reach personal goals and become more independent Communicate clearly, professionally, and compassionately Take initiative and use sound judgment when working independently Value collaboration and contribute positively to a team Are receptive to feedback and interested in learning and professional growth Can balance compassion with appropriate professional boundaries Qualifications Experience supporting individuals with intellectual or developmental disabilities, autism, mental health needs, or learning disabilities is preferred Strong communication, organization, and interpersonal skills Ability to work independently while remaining connected and accountable to a collaborative team Availability to work at least one weekend day each week Valid California driver's license and a clean driving record Ability to pass a Department of Justice background check Ability to complete a physical examination and TB testing Adult CPR and First Aid certification, or willingness to obtain certification after hire About Adaptive Learning Center Founded in 1988, Adaptive Learning Center is a nonprofit organization based in Concord, California.
