Job Description

Case Manager - Cal
AIM / ECM
Programs We are currently seeking compassionate and motivated Case Managers to support high-need populations through CalAIM initiatives, including Enhanced Care Management (ECM) and Community Supports programs. In this role, you will work directly with clients to coordinate care, connect individuals to housing and healthcare resources, and provide ongoing support to improve stability and long-term outcomes. This is an excellent opportunity for someone passionate about community impact, advocacy, and serving underserved populations.
Key Responsibilities:
  • Conduct client assessments and develop individualized care plans
  • Manage a caseload of approximately 25-60 clients
  • Coordinate housing, medical, and behavioral health services
  • Perform field outreach, including home visits and community-based support
  • Maintain accurate documentation and ensure compliance with program requirements
  • Collaborate with internal teams and external providers
  • Provide crisis intervention and ongoing client advocacy
Qualifications:
  • Bachelor's degree in Social Work, Psychology, Public Health, or related field preferred
  • 2-3+ years of case management or related experience
  • Experience working with underserved or high-need populations strongly preferred
  • Familiarity with CalAIM and ECM programs is a plus
  • Valid driver's license required for field-based work
Compensation:
  • $23.00 - $28.00 per hour DOE
