Job Description

Purpose:
Case Manage for Developmentally Disabled Adults that live in a family home. Time spent in the office and time spent driving/at client home is about 60/40. Must have a Driver's License, personal vehicle and be willing to drive to multiple homes throughout the week. Job Duties (including, but not limited to): Provide for timely case coordination with current case load by ensuring home visits are conducted according to Program requirements, prepare monthly reports, assist Providers as necessary, and participate in quarterly and monthly meetings as required. Provide for timely communication by ensuring attendance at required meetings, answering and responding to Provider questions, and providing timely feedback to Social Workers and/or Counselors. Ensure all documentation is prepared in accordance with requirements by reviewing medication logs, consumer documentation and/or other Agency reporting. Represent the Company on a professional level ensuring all verbal and written communication is of a professional nature, correct grammar, and that written documents are reviewed for accuracy prior to submitting. Other duties and special projects as assigned. Bachelors degree preferred but not required.
