Job Description
Job Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. We are looking for someone who has experience working with individuals with intellictual disabilities. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients. You will be working in the field with the individuals and there family.Responsibilities:
- Conduct comprehensive assessments of client needs individuals that have Developmental Disabilities and develop individualized care plans
- Coordinate and monitor services to ensure clients receive appropriate support
- Provide counseling and guidance to clients dealing with addiction, behavioral health issues, or crisis situations
- Collaborate with other professionals, such as social workers, healthcare providers, and community organizations, to coordinate services
- Advocate for clients' rights and access to resources
- Supervise and monitor client progress towards goals
- Maintain accurate and up-to-date documentation of client interactions and progress
Experience:
- Bilingual is a plus
- Bachelor's degree in social work or related field
- experience working with indiviudals with special needs (Autism/Downs Syndrome)
- Previous experience in case management or related field
- Knowledge of crisis intervention strategies
- Familiarity with Section 8 housing programs, Medicare, child welfare systems, and workforce development resources and all generic resources
- Strong communication skills and ability to build rapport with diverse populations
- Proficient in motivational interviewing techniques
- Ability to work independently and as part of a team
