Non-Exempt Salary Range:

Position Summary The Case Manager II provides rehabilitation, care coordination, and supportive services to adults receiving outpatient behavioral health services while promoting wellness and recovery. Key Responsibilities Provide rehabilitation services, coordinate care with treatment teams, conduct community-based services and home visits as appropriate, maintain documentation standards, collaborate with community partners, and support recovery-oriented treatment approaches. Required Qualifications Qualified candidates must meet one of the education and experience combinations identified by the position requirements and possess a valid California Driver License with an insurable driving record. Preferred Qualifications Behavioral health experience, knowledge of community resources, and experience working with adults experiencing mental health or co-occurring disorders are preferred.
