Description SUMMARY The San Francisco Homeless Outreach Team (SFHOT) works collaboratively in small teams to engage and stabilize individuals experiencing homelessness, to help establish treatment, and to find permanent housing. A Case Management Level 1 is an entry level position on the case management team. A Case Management Level 1 manages their caseload, as well as supports the functions of HOT as a whole. The work schedule is Monday-Friday, 8am-4:30pm. Schedule is set by management. This position is represented by a union and is covered under the terms of the union's Collective Bargaining Agreement. Essential Functions Engage individuals and assess needs (e.g., medical, mental health, substance use, shelter, food access, benefits, and other issues). Follow up with clients to support linkage, as needed. Perform wellness checks, assess risk, provide support, and referrals for needed This may include crisis intervention, engaging emergency medical or mental health systems, or mandated reporting. Advocate and collaborate with service providers to facilitate access to care with the goal of stablization. Maintain a minimum caseload of 15-20 clients that present with complex medical and/or behavioral issues, develop service plans, initiate referrals and linkages, and stabilize into permanent Develop engagement strategies and interventions. Respond to requests from city agencies (the Mayor's Office, Board of Supervisors, SFPD, SFFD, DEM, DPH, HSH), merchants, community groups, and citizens to help individuals found at-risk in places not meant for human habitation. Work collaboratively as part of a dynamic and diverse team, which includes clear communication, flexibility, and humility. Work in close collaboration with the DPH Street Medicine Team to link people experiencing homelessness linking to primary care. Maintain professional and positive relationships with other service providers, community groups, and the neighborhood. Engage and participate in clinical supervision, both group and individual, to discuss client care, service delivery, and clinical and professional development. Use assigned communication devices to appropriately and professionally communicate with team members and other service providers. Use web-based technologies to look up information and collateral data relevant to service delivery. Maintain timely and accurate documentation according to program requirements. Use company provided vehicles in an appropriate and professional manner. Maintain a clean driving record. Adhere to the Code of Conduct as well as established policies and procedures. Attend all mandatory trainings and meetings. Other duties as assigned.MINIMUM QUALIFICATIONS TARGET EXPERIENCE
Direct experience working in a professional, volunteer, or other relevant capacity with people experiencing homelessness or individuals with complex medical and/or behavioral health concerns. All areas of study meet qualifications.The following majors are preferred:
Social Work, Human Services, Psychology, Counseling, Sociology, Anthropology, Ethnic Studies, Public Policy, Public Health, or a related field. High School degree or equivalent plus 2 years or more with TARGET EXPERIENCE, or Specialty Certificate related to the populations served (e.g., Community Health Worker, Medical Assistant, Drug and Alcohol/CADAC, Peer Counselor) and 1 year withTARGET EXPERIENCE
; or AA/AS degree, AND Valid California driver's license. Driver must be 21+ with none of the following in the past 3 years: any 2-point convictions; or more than (2) moving violations; or more than (1) moving violation and (1) at-fault or a passenger handling accident. Basic knowledge of the client population and their complex needs including homelessness, financial instability, medical and mental health, and substance abuse. Ability to work well with diverse staff and clientele including cultural, language, sexual identity, gender, and other diversity considerations within all neighborhoods in San Francisco. Ability to work successfully both independently and coopratively. Ability to speak, read, and write clearly in English. Basic level competency using the Internet, Outlook email, and Microsoft Word computer applications as well as ability and willingness to learn necessary programs needed for everyday job function. Ability to use computers/internet to look up information and enter relevant notes. CPR and first aid certification within 90 days of hire.PREFERRED QUALIFICATIONS
Speak other languages in addition to English. Previous experience or training in street outreach and case management. Knowledge of San Francisco neighborhoods and community resources. Funding is by LA Care's Generating African American Infant & Nurturers' Survival Initiative (GAAINS II). This is a hybrid virtual and in-person position located in Los Angeles County, California. Applicants must reside in Los Angeles County, California. This project will provide postpartum doula in-home supportive services and support by one (1) Certified Postpartum Doula for eight (8) African American mothers and infants through a home visitation program for clients and their infants from two days postpartum through three months postpartum. Families will be able to have up to 24 hours of postpartum care in their homes (4-hour or 8-hour sessions) free of charge if they meet eligibility criteria (African American, residing in Los Angeles County, and have Medi-Cal). Breastfeeding will be promoted by doulas, with at least 70% (7 of 10) of clients, using this feeding method for a minimum of six months. Also, assessment for perinatal mood and anxiety disorders (PMADs) will be conducted and referrals shall be made for mental health services if needed. About 4-6 hours a week during Monday-Fridays from 9:00 am to 5:00 pm will be spent on outreach to identify and enroll eligible clients for the program through social media marketing on Instagram, Twitter and Facebook, as well as participating in community meetings in-person, doing doctor's office and clinic visits and participating virtually over Zoom or Teams meetings with partner agencies. Occasional outreach at health fairs on weekends is also required. Reliable transportation and personal computer are required. Interested applicants please submit a resume highlighting relevant work as a graduate of a Postpartum Doula Training Course, or perinatal home visitor. This is a temporary part time position (12.5/hours week) funded through June 30, 2026.ESSENTIAL FUNCTIONS
Understanding of quality-of-life issues and the impact of social determinants of health and racial justice issues impacting birth outcomes and survival rates of African American mothers and infants. Ability to provide culturally congruent services to African American mothers and their nurturers or fathers of the baby and make referrals to African American fathers to our Fatherhood Support Program Coordinator. Work with Program Director to implement a plan to enroll eight (8) African American mothers and their infants who are between two days and three months postpartum into the program provide in-home supportive services during the two-day to three- month postpartum period. Under supervision of Program Director, implement strategies to ensure at least 70% (5 out of 8) of African American mothers participating in the program will breastfeed their babies for a minimum six months during the child's first year of life. Implement plan to deliver strategies to ensure at least 95% (7 out of 8) of African
- American mothers and their babies participating in the program will survive through one year post birth.
JOB QUALIFICATIONS
Graduate of a Postpartum Doula Training or Birth Doula Training having worked with minimum three mother-baby dyads during the first 8 weeks after birth; Certified Postpartum Doula preferred. Certification as a Lactation Educator or Lactation Counselor-Educator or Breastfeeding Peer Counselor through any qualified organization such as CAPPA, Breastfeed LA, Le Leche League, WIC, UCLA, etc. preferred; Six months experience working as a postpartum doula or in-home health visitor providing social services to a diverse population including Black/African American families. Demonstrated ability to implement research-based health education strategies and best practices in perinatal health and lactation. Ability to plan, coordinate, and organize work projects and meet deadlines. Strong interpersonal communication skills and conflict resolution ability. Cultural awareness training and anti-racism training or willingness to attend training within 1 month of hire. Experience collaborating with a variety of staff at all levels and community partners. Strong attention to detail to complete online visit forms by deadlines; Passion for and commitment to the mission of Happy Mama Healthy Baby Alliance Aptitude for solving problems with creativity and resourcefulness. Ability to develop positive relationships with clients, doulas, providers, staff, interns, apprentices, volunteers, and supervisors. Ability to work independently with little supervision, keep up with required documentation and reports and meet deadlines for submitting reports. Ability to work flexible hours (including nights, weekends, holidays), work virtually, and have reliable transportation and the ability to travel throughout Los Angeles County. Completion of HIPAA Training within 1 week of hire required. Training in Perinatal Mental Health required or willingness to be trained within 1 month of hire required. Computer proficiency with Microsoft 360 Programs such as Word, SharePoint, Excel, PowerPoint, and possession of a personal computer are required. Education/Experience High School Degree (required); or associate's degree (preferred) or college-level coursework in subject areas of women's studies, health sciences, public health, social work, family studies, obstetrical and postpartum nursing, or midwifery. Demonstrated experience in postpartum care of mothers (experience via parenting and/or early learning and development and home visitation programs is highly desirable) as a Postpartum Doula and/or Midwife Assistant. Six months experience working as a Postpartum Doula or in-home visitor providing social services and lactation support to diverse populations including BIPOC, refugees and immigrants, including adolescents. Certificates/Licenses/Clearances Graduate of a Postpartum Doula or Perinatal Support Specialist Training required. Certified Postpartum Doula from any certifying entity preferred. ESSENTIAL FUNCTIONS
§ Supervision of adult homeless clients in an emergency shelter in terms of access to safe housing, meals and services. § Intake of adult applicants to assess housing need, support and benefits services. § Develop and manage emergency shelter day-to-day services. § Supervision of engagement activities designed to build a healthy and safe environment for sleeping, eating, and living in a group setting. § Communication, both verbal and written, with adult clients, staff, law enforcement and others to determine situational shelter needs. § Documentation in case records, and files to ensure the written record of work performed on behalf of clients, including completion of incident reports. § Light housekeeping/general clean up. § Ability to handle emergency incidents, including medical, mental health, violent and/or other unanticipated events, including evacuation due to earthquake, chemical release, incarceration, or other incidents. § Conducts and documents hourly site walkthroughs inside and outside to ensure a safe and comfortable living safe for staff and residents § Always keeps a clean and welcoming facility by doing light housekeeping/general clean-up of all inside and outdoor spaces; stocks up program supplies as needed § Reports and seeks immediate assistance during any unsafe or unusual occurrences; in the absence of onsite management the Shelter Specialist contacts on-call management during evening and weekend hours § Attend all mandatory meetings and trainings § Other duties as requiredJOB QUALIFICATIONS
