Full Time Job Summary:
Under the direct supervision of the Director of Operations, the case manager works with and manages client cases from intake to discharge, follow client progress, investigate client grievances, keep Program Manager informed of any issues/problems with any clients.Duties and Responsibilities:
Complete intakes Contact consumer CSC Schedule intakes and assessments Determine level of support for consumer Meet with team to discuss services for new consumers Schedule all IDT meeting and complete documentation Complete all ISPs for consumers (30-day assessment; semi-annual; annual reports) Meet with all consumers quarterly to discuss any changes to their services, address any issues. Write and submit all SIRs with the approval of manager Meet with consumers as needed to address any issues with services and determine if changes need to be made And other duties as assigned Minimum Qualifications- High school diploma
- Minimum 2 years hands-on experience working with individuals with a wide range of intellectual and developmental disabilities (group home or day program experience)
- Knowledge working with Regional Center
- Detail-oriented, ability to multi-task and manage time well
- Excellent interpersonal skills
- Ability to diffuse crisis situations and apply problem solving strategies
- Acceptable driving record
- Ability to travel within Riverside County where services are provided
- Successful completion of background check and TB test
