Position Type:
Part-Time (with potential to transition to Full-Time)
Compensation:
Pay based on experience Phoenix Transitional Housing Plus is seeking one compassionate, motivated Part-Time Case Managers to support our AB12 clients in Lompoc, CA (Santa Barbara County). These roles are crucial in helping young adults transition to independent living through personalized case management and consistent support.
Minimum Requirements:
Bachelor's degree in Behavioral Science or related field (required) Clean driving record (required) Reliable transportation and proof of car insurance (required) Strong communication and documentation skills Ability to work independently and maintain professional boundaries
Preferred Qualifications:
Experience working with transitional age youth (TAY) or foster youth Knowledge of community resources in Tulare County Bilingual (Spanish/English) a plus
Job Responsibilities:
Provide trauma-informed case management to assigned clients Support clients in achieving goals related to education, employment, housing, and independent living skills Coordinate and attend appointments, meetings, and resource referrals Maintain accurate and timely documentation Collaborate with internal staff, community agencies, and service providers This is an excellent opportunity for someone passionate about supporting youth and young adults in transition.
