Description Case Manager Brief Job Description:

We are looking for a Case Manager to coordinate services, support clients, and monitor progress toward program goals.

Responsibilities:

+ Conduct client assessments and develop service plans + Connect clients with community resources and support programs + Maintain detailed case notes and documentation + Monitor client progress and follow up on services provided + Collaborate with internal staff and external partners

Requirements Requirements:
