In-Home Outreach Counselor FLSA Status :
Non-Exempt In Home Outreach Counselor will contribute to the goals and objectives of Tessie Cleveland Community Services Corporation (TCCSC) and its mission assessment, develop comprehensive treatment plans and arranging for services and activities. This position will report to the Clinical Director for the designated area office. The counsellor participates in the quality assurance of agency documentation.
Position Qualifications/Requirements Job Requirements:
Must have one year of direct experience with Family Preservation Programs in an in outreach in home health care setting. Knowledgeable in areas counselling in child safety, case management, crisis intervention and advocacy services. Must possess basic computer skills. Must be able to read, write and speak English fluently. Must meet Company requirements of insurability by the agency insurance carrier(s) to be insured under the Agency's policy.
Credentials Required:
Bachelor's degree in psychology, social work or marriage and family therapy from an accredited college or university is required. Must be an MSW, MFT or PCC at the BA level under the supervision of LCSW or LMFT licensed.
Physical Demands:
While performing the duties of this job, the employee regularly operates a computer and other office machinery. The employee frequently moves about inside the office and outside and constantly positions self throughout performing job duties. Occasionally lifts and carries stacks of documents weighing up to 25lbs. Communicates with others in the office environment and outside the office environment. The employee works in the field and office settings and must be flexible to requirements.
Work Environment:
Office and fieldwork Duties and Responsibilities Provides assessments and develops comprehensive treatment plans and arrange for services and activities. Conducts face-to-face weekly counselling visits. Monitor child safely, provide linkage, case management, crisis intervention and advocacy services. Document sessions through progress notes and maintain them in the case record. Complete monthly progress records and record the m in the Multi-disciplinary Planning Case Planning service plan and monthly progress report. Utilizes the Shared Core Practice model to deliver services that incorporate the guiding principles of engaging, teaming, assessment, planning and intervention, and tracking and adapting. Participates in multi-disciplinary team conference to discuss and evaluate the nature of the client's problem, to evaluate the client's progress, and to develop disposition plans. Attends all mandatory meeting, including staffing's, group supervision and be able to participate, facilitate and/or teach if needed. Completes and submits all documentation timely, accurately, and according to TCCSC policies and procedures, as directed by supervisor. Responds to emergencies utilizing crisis intervention techniques to deescalate individual and/or family situations. Participates in quality assurance and continuous quality improvement. Be familiar with and adhere to all agency policy and procedures. Will adhere to other duties assigned by supervisor, manager and/or coordinator.
