This is a fully onsite position focused on helping residents maintain housing stability through ongoing case management, crisis intervention, resource coordination, and connection to community services. Responsibilities Manage a caseload of residents in Permanent Supportive Housing. Conduct regular resident check-ins and assessments. Connect residents to mental health, healthcare, benefits, employment, and community resources. Coordinate services with behavioral health providers and property management. Provide crisis intervention, de-escalation, and housing retention support. Maintain accurate case notes, service plans, and program documentation. Participate in team meetings, trainings, and collaborative care planning. Qualifications Minimum 2 years of case management experience working with homeless or vulnerable populations. Experience supporting individuals with mental health, substance use, or co-occurring disorders. Knowledge of Housing First, trauma-informed care, motivational interviewing, and harm reduction practices. Strong communication, documentation, and problem-solving skills. Ability to work independently in a fast-paced supportive housing environment. Preferred Experience in ICMS, PSH, homeless services, behavioral health, or supportive housing programs. Familiarity with HMIS or other case management systems. Bachelor's degree in Social Work, Human Services, Psychology, or a related field. Onsite in Pomona, CA | 3+ month contract covering for
