Our mission is to help homeless youth and at-risk families become productive and integrated members of the community. We provide emergency and permanent housing options and community-based services to support long-term stability and independence. The College Success Initiative (CSI) serves college students experiencing homelessness or housing insecurity across campuses in Los Angeles and San Bernardino Counties. CSI supports students in achieving educational success while helping them access and maintain safe and stable housing. Position Summary The Case Manager provides direct services and housing support to college students experiencing homelessness or housing insecurity. This includes comprehensive case management, coordination with campus partners, and support navigating housing systems. Jovenes is currently hiring for Case Management positions that will work with students from CSU-San Bernardino and San Bernardino Valley College. Key Responsibilities Student Engagement & Case Management Conduct outreach and intake for referred students Complete assessments of housing and supportive service needs Develop individualized Housing Service Plans (HSP) and update monthly Provide case management, referrals, and crisis intervention to support academic and housing goals Support students in accessing mental health, substance use, medical, and other supportive services Assess and document financial need; make funding recommendations aligned with HSP Monitor and support students' progress through consistent follow-up Housing Navigation & Program Support Assist students in identifying and securing permanent or temporary housing Provide housing navigation and co-located support services on campus Oversee master lease or rapid rehousing agreements when applicable Maintain and clean emergency housing units as needed Conduct move-ins/move-outs and support students with transporting belongings Track and submit rental subsidy payments and requests Perform physical tasks such as moving boxes/furniture, lifting up to 20 lbs, deep cleaning, and organizing student living areas as needed ( Reasonable accommodation for individuals with disability will be evaluated and exceptions considered) Collaboration & Coordination Coordinate closely with campus staff and other service providers Participate in regular case conferencing and team meetings Delegate and assist with appropriate tasks with other team members to enhance student outcomes Documentation & Compliance Maintain complete and up-to-date participant records in both physical files and electronic systems Submit reports and documentation per program and funder requirements Track and reconcile program-related expenses in accordance with agency procedures Maintain confidentiality and compliance with agency standards and applicable laws
Skills & Qualifications Required:
College Degree (AA or BA) 2-3 years of experience in case management or related direct service Experience working with transitional age youth, low-income communities, or individuals with mental health challenges Strong written and verbal communication skills Ability to work independently, manage time, and balance multiple priorities Valid CA driver's license and access to an insured personal vehicle
Preferred:
Bilingual (English/Spanish or other languages) Experience with college students, housing navigation, or homeless services Familiarity with trauma-informed care and harm reduction models Work Conditions & Expectations Regular travel to assigned campuses, student housing locations, and Jovenes offices is required; mileage reimbursement is provided Case Managers may be reassigned to different campuses or programs as needed Case Managers may be assigned additional duties and responsibilities according to program and student needs Some evening or weekend availability may be required based on student and program needs Must uphold Jovenes' code of conduct and maintain ethical and professional standards Benefits Health and dental insurance provided Opportunities for professional development Supportive and mission-driven work environment This is a full-time position. Hourly range:
$23.00 - 26.00. Health, dental and life insurance provided, complementary vision insurance available. Paid time off, sick time and paid holidays are provided according to agency policies. Employee will be able to enroll in agency's 401k plan. Agency provides matching contributions according to agency policies. Must have your own car.
