A culture that celebrates and rewards your hard work and dedication What you bring to SBHG (qualifications) : Education and Experience (Applicants should meet any one of the following experience and education combination) 4 years of experience in a social service field OR Associate degree plus 2 years of experience in the social service field OR Bachelor's degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field Licenses and Certifications Valid California Driver's License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy. How you will make a difference (job overview): This position has primary responsibility for case management services to clients and their families referred through Stars' Transition Age Youth (TAY) Program (TAY).
Division/Program Overview:
Outpatient, community-based program that serves youth age 18-25 with serious and persistent mental health issues Program provides intensive case management, outreach and engagement, medication support, as well as mental health counseling, peer groups, and supportive services to assist TAY transition to adult life.
