ECM Field Case Manager Alinea Health San Luis Obispo, CA Job Details Part-time $30 an hour 1 day ago Benefits Travel reimbursement Mileage reimbursement 401(k) Qualifications Driver's License Medical records Case management Medicaid Social Work Full Job Description About the Role Alinea Psychology Inc. is expanding our Enhanced Care Management (ECM) program under CenCal Health and is seeking a Part-Time Field-Based ECM Case Manager to provide in-person care management and outreach to Medi-Cal members across San Luis Obispo county. This role is primarily community-based , involving home visits, meetings in community settings, and outreach to hard-to-reach members. This is an excellent opportunity for professionals who want meaningful field work with flexibility, or who are looking to add ECM work alongside another role. What You'll Do Conduct in-person outreach to assigned CenCal ECM members (home visits, shelters, community sites, hospitals, and public locations) Complete ECM enrollment activities , including consent, assessments, and care plan development Serve as the primary point of contact for members once enrolled Coordinate care across medical, behavioral health, housing, social services, and community resources Support high-need populations, including individuals experiencing housing instability and frequent emergency utilization Maintain timely and accurate ECM documentation in compliance with CenCal and Medi-Cal requirements Collaborate with internal clinicians, supervisors, and outreach staff Track outreach attempts and outcomes in accordance with billing and compliance standards Travel throughout the CenCal service area as needed (mileage reimbursed) Who This Role Is For You are comfortable working face-to-face in the community , not in an office You're confident engaging clients who may be difficult to reach or reluctant to trust systems You're seeking a part-time role with meaningful client impact You can work independently while staying organized, accountable, and compliant Required Qualifications Experience in case management, care coordination, outreach, or community health Familiarity with Medi-Cal populations (ECM, care management, behavioral health, social services, or similar) Reliable transportation and valid driver's license Ability to travel locally for in-person visits Strong documentation and organizational skills Comfort using electronic health records or case management systems Preferred Qualifications Previous experience with Enhanced Care Management (ECM) or managed care Background in behavioral health, social work, public health, or human services Experience working with individuals facing housing instability or complex social needs Bilingual (English/Spanish) strongly preferred Schedule & Compensation Part-time schedule (weekday availability required) Competitive hourly pay based on experience Mileage reimbursement for field travel Strong administrative and supervisory support Opportunity to transition to a full-time role as the ECM program expands Why Alinea Psychology We are a growing, mission-driven healthcare organization building high-quality ECM programs at scale .
