Under general supervision, the Support Service Worker II (Home Visitor Assignment) performs duties to support families through relationship-based home visiting using the Healthy Families America (HFA) Best Practice Standards. The home visitor builds and sustains trusting relationships with families, engages them in goal setting, and provides education, information, referrals, and service coordination to support family well-being and child development.
The Support Service Worker II conducts strength-based home visits informed by parent-child observations and standardized developmental screenings, assists families in understanding typical child growth and development, and supports parents in identifying goals and next steps. The position coordinates services with community partners and public benefit programs, acts as a liaison between families and service providers, and supports families in accessing preventive health care and community resources.
This role is also responsible for maintaining accurate, detailed, and timely documentation in accordance with program and County requirements, including case notes, assessments, and reports. The Support Service Worker II works collaboratively with a multidisciplinary team, participates in reflective supervision, and contributes to outreach and recruitment efforts to engage families in services.
There is currently one (1) regular, full-time Support Service Worker II vacancy in the Healthy Families Marin Home Visiting Unit.

The Support Service Worker II reports to a Public Health Unit Supervisor and is part of the Healthy Families Marin Home Visiting Unit within the Division of Public Health in the Health and Human Services Department at the County of Marin. Healthy Families Marin partners with prenatal individuals, new parents, families, and children from birth to age five to strengthen the parent-child relationship and promote healthy child development.
