Case Manager
  • Homeless Services / Interim Housing Holliday's Helping Hands is seeking compassionate, reliable, and experienced Case Managers to join our team and support individuals experiencing homelessness.
Our mission is to help end homelessness by providing interim housing, supportive services, nourishing meals, and safe environments that empower our participants on their "Journey to Home." Case Managers play an important role in helping participants identify barriers, establish personal goals, connect with community resources, and work toward housing stability and permanent housing . Core Responsibilities Complete initial needs assessments and respond to critical needs in a timely manner. Develop comprehensive, client-driven case plans with short
  • and long-term goals.
Provide ongoing case management, advocacy, and service coordination. Build supportive relationships with participants and encourage engagement in services. Assist participants with identifying and overcoming barriers to housing stability. Connect participants with healthcare, mental health, substance use, employment, public benefits, and housing resources. Assist with affordable housing applications and housing navigation. Provide outreach, engagement, follow-up, and supportive services. Monitor participant progress and update case plans as needed. Maintain accurate, timely, and confidential case documentation. Collaborate with healthcare providers, housing providers, community organizations, and other service providers. Provide crisis intervention and appropriate referrals when necessary. Assist participants with public benefits applications and other supportive services. Other duties as assigned. Minimum Qualifications
  • Funder / County Requirements Case Managers must meet at least one of the following qualification pathways: Option 1
  • PEH Experience + Bachelor's Degree At least one (1) year of experience working with PEH (People Experiencing Homelessness); AND A Bachelor's degree from an accredited college or university in Social Work, Mental Health, or a related field. OR Option 2
  • Direct Mental Health / Related Services A minimum of two (2) years of experience providing direct mental health services or services similar to those described in the applicable Scope of Work (SOW), unless otherwise approved by the County; AND Experience working with participants experiencing: Mental illness Chronic health issues Substance use disorders Candidates must also demonstrate knowledge or expertise in: Chronic homelessness Outreach and engagement strategies Intensive case management services Best-practice models Mental health services Substance use disorder services Crisis intervention Suicide assessment and prevention Affordable housing and housing navigation Public benefits applications Housing and landlord/tenant rights Eviction prevention Client advocacy and service coordination Preferred Qualifications Experience working as a Case Manager or in a similar direct-service position.
Experience working with People Experiencing Homelessness (PEH). Experience in interim, transitional, recuperative, supportive housing, or shelter environments. Experience serving individuals with mental health, chronic health, or substance use challenges. Knowledge of Los Angeles County homeless services and community resources. Bilingual Spanish/English preferred. Strong written and verbal communication skills. Strong documentation and organizational skills. Ability to maintain professional boundaries and confidentiality. Work Environment This position works directly within an interim/transitional housing environment serving individuals and families experiencing homelessness. The Case Manager must be comfortable working directly with participants, collaborating with multidisciplinary teams, responding to changing client needs, and maintaining a safe, respectful, and supportive environment.
