Per Hour Schedule:
6 hours on Tuesday, Wednesday and Thursday between the hours of 8:00am to 4:30pm/open to discuss hours MHM Services, a Centurion company, is proud to be the provider of mental health services to the California Department of State Hospitals Conditional Release Program (CONREP). The CONREP program is a statewide system that provides standardized intensive community outpatient mental health services for judicially committed individuals and mentally disordered offenders. We are currently seeking a PRN/Per Diem Socialization Coordinator/Case Manager to join our team at our Sonoma/Solano CONREP program located in Vallejo, California. The Socialization Coordinator/Case Manager provides community-based services and monitoring to clients that are judicially committed in addition to mentally disordered offenders. The Socialization Coordinator is responsible for providing individualized recreational and socialization support and services as well as psychoeducation to address coping, life, job, self-care, and social skills.
