Kings View is seeking a Case ManagerI - CARE
to support the CARE Court program. This role provides community-based case management and linkage services to individuals experiencing severe mental health and substance use challenges. The Case Manager works closely with the CARE Court team and community partners to support engagement, coordinate care, and assist individuals in accessing services that promote stability, recovery, and improved quality of life.Key Responsibilities:
Care Coordination & Case Management:
Provide case management services to individuals with mental health, substance use, and co-occurring conditions Develop and coordinate comprehensive care plans Facilitate linkages to community resources, housing, and support services Monitor client progress and communicate with treatment teamsCARE Court & Community Collaboration:
Participate in CARE Court hearings and collaborate with justice system partners Work closely with outreach teams and clinical staff Coordinate with community agencies to support continuity of careClient Support & Skill Building:
Assist individuals in developing daily living, social, and independent living skills Provide support with hygiene, meal preparation, and functional skill development Offer medication education and support resource navigationDocumentation & Compliance:
Maintain accurate and timely documentation in SmartCare EHR system Ensure compliance with agency, HIPAA, and program standardsCrisis Awareness & Safety:
Respond appropriately to individuals experiencing behavioral health crises Utilize de-escalation techniques and follow safety protocols Coordinate with emergency services when neededGeneral Duties:
Collaborate with multidisciplinary teams Support program design and implementation Work independently while maintaining strong communication with supervisors Perform additional duties as assignedWhat You'll Bring:
Required Qualifications:
High School Diploma or equivalent One (1) year of related experience Valid California Driver's License (Class C)Required Experience:
Experience in behavioral health, social services, or related field Lived experience or familiarity with mental health, substance use, homelessness, or traumaRequired Skills:
Strong communication and interpersonal skills Ability to provide case management and linkage services Strong documentation and organizational skills Ability to build relationships and work collaboratively Ability to think critically and solve problems independentlyPreferred Qualifications:
College coursework in a related field Bilingual English/Spanish preferred CNA or Medical Assistant experienceProfessional Attributes:
Compassionate and empathetic Patient and persistent Strong time management skills Ability to work with diverse populations Positive attitude and strong teamwork skillsAdditional Requirements:
