Case Manager. Job Description This position supports the work of the Y, a leading nonprofit, charitable organization committed to strengthening community through youth development, healthy living and social responsibility. The
CHAP/CHEER
Case Manager at the Greater Waterbury YMCA provides case management for clients in the
CHAP/CHEER
program. Qualifications Bachelor's Degree in closely related field (Social Work, Secondary Education or Counseling) Experience in teaching and reinforcing life skills and assistance with community linkages for development of natural supports The ability to work professionally with others, as well as autonomously Minimum of two years of experience working with youth. Excellent interpersonal communication and writing skills, well-organized, the ability to manage your time effectively, and operate business computer applications at an intermediate level (Microsoft Word, Excel, Business Email Systems, etc.) Must have and maintain a clean CT Driver's License, current state registration and be the policy holder of full auto insurance coverage Essential Functions Provide 5-40 hours of case management services Must have safe, reliable vehicle. Mileage reimbursement paid monthly. Teach and reinforce all life skills documented on the Life Skills Inventory checklist, including budgeting, housing, education and employment Be knowledgeable of community resources and provide information and assistance with community linkages for development of natural supports Assist clients with finding and enrolling in suitable secondary education and obtaining financial aid Monitor educational status including attendance and grades Assist clients with preparing a resume and job searches Assist new clients and those wishing to move with locating and maintaining housing Maintain knowledge about program guidelines and DCF policies and procedures Ensure required documentation and reports are completed and presented as required
