Through our SSG Fox Project and Veterans Rally Point initiatives, we provide vital support services and resources to Service Members, Veterans, and their families (SMVFs) while working to prevent veteran suicide through community engagement and direct intervention. What's in it for you? We are seeking a dedicated Case Manager to join our Military Services team. This role offers the opportunity to provide crucial support and care coordination for veterans and their families while working with a collaborative team focused on crisis intervention and suicide prevention. Behavioral Health Assessment and Care:
Conduct suicide and risk assessments Facilitate initial care assessments to determine treatment priorities Create and manage Individualized Service Plans (ISPs) Focus on skill-building components including problem-solving, positive activities, managing reactions, helpful thinking, and social connections
Care Coordination:
Coordinate referrals and facilitate warm handoffs to VA facilities Ensure seamless transition and continuity of care Build relationships with service providers Develop and maintain community partnerships Travel required, reimbursement for mileage
Community Engagement/Outreach:
Attend local veteran events Represent Easterseals and Veterans Rally Point in the community Facilitate outreach activities for SSG Fox building partners and referral networks Maintain highest ethical standards in all interactions Support special projects as needed Skills and requirements Bachelor's Degree in Social Work or relevant Case Management experience required Military service or experience strongly preferred Minimum 2 years direct Case Management experience Experience in Crisis Intervention and Suicide Prevention programming required Eligibility to become a Peer Support Specialist Strong background in care coordination Experience with community outreach and relationship building required Excellent project management and organizational abilities Strong conflict management and trauma-informed de-escalation skills Proficiency with Microsoft Office Suite Self-motivated with ability to work independently and collaboratively Strong attention to detail while managing multiple priorities Valid driver's license with proof of insurance; willingness to use personal vehicle Flexibility to work early mornings, evenings, and weekends as needed Commitment to maintaining confidentiality and professionalism Passion for Easterseals' mission and ability to serve as an organizational ambassador
