We are a Housing First organization providing Permanent Supportive Housing (PSH) Case Management services for adults experiencing homelessness. The PSH Case Manager on our team in Washington DC team will:
  • provide community-based direct services that promotes the recovery of individuals experiencing homelessness.
  • engage in on-going clinical assessment and formulates accurate diagnostic impressions
  • partner with individuals and families to create goals and develop the Individualized Service Plan
  • provide housing navigation services for participants weekly; of which a minimum of two per month will be face-to-face
  • Assists participants in requesting furnishings and housewares and coordinates/supports move-in
  • help clients develop skills that promote stability, employment, housing retention and community engagement
  • Promotes and supports individual's/families' recovery in a manner that increases skills, sobriety, psychiatric stability, housing stability, and meaningful community engagement.
  • collaborate, coordinate and advocate with other service providers
  • document case management activities in HMIS, HTH and other DHS mandated reporting systems Required qualifications : Bachelor's degree in social work, counseling, psychology or related field.
Minimum one year of case management experience working with individuals with co-occurring mental health and substance use disorders, and/or experiencing homelessness. Four years of experience or certification in a relative discipline may substitute for the Bachelor's requirement. Must have a valid drivers license and reliable vehicle. Must be able to pass a driving record and criminal background check. Hiring preference will be given to residents of the District of Columbia.
