Description:
Responsibilities include but are not limited to: Conducting intakes and needs assessment Create individualized service plans Provide individualized support to program participants Ensure all program participants have necessary personal documentation including but not limited to insurance enrollment, food stamps, identification, social security card, and other social service benefits; Assist clients with any and all legal matters Connect program clients with the appropriate resources prior to discharge Develop community resource network Prepare and distribute electronic reports on client data and progress with services needed Plan and secure discharge proposals with follow up, in coordination with clients and management Conduct educational groups with program participants Documenting in electronic medical record Data Collection and Reporting as assigned Transport clients to community agencies as assigned Incumbent will focus on wraparound services, including case management, housing assistance, access to healthcare, and other essential resources, complements the existing continuum of care. It enhances the support available to individuals by addressing the various needs and challenges they may face beyond addiction treatment. This comprehensive approach helps individuals maintain recovery and provides a safety net to prevent relapse. Incumbent will actively collaborate with community organizations, such as nonprofit agencies, housing providers, and educational institutions, to enhance the continuum of care. This collaboration strengthens the network of services available to individuals in need of addiction services and fosters a community-wide approach to supporting recovery and long-term well-being.
Requirements:
Experience in case management Knowledge of wraparound services Possess strong interpersonal and communication skills Ability to present educational topics in group format Ability to develop community resource network Computer skills required including Microsoft excel, word, power point and online electronic medical record Must have valid driver's license and submit DMV record at time of hire Exhibit organizational and time management skills
