Job Description

Reports To:
Case Management Director/ Executive Director Purpose:
Manage Love INC client cases and effectively track and follow client case plan. Generate required reports, including for grants and other crisis spending and report on all outcomes and metrics. Extend the mission and awareness of Love INC to the East side of Sussex County.
Hours:
40hrs 8:30am-4:30pm in office
Exempt Responsibilities:
1. Understand and be able to conduct full client intake, verification, referral tracking and follow-up of clients. 2. Understand and help create and train best practices for other connection center workers to use to effectively follow changing protocols surrounding grants and crisis funds tracking. This includes individual Church Partner and Church Ministry processes. 3. Track and report spend downs for grants for crisis funds. 4. Review information gathered by volunteers during the intake and analyze. 5. Determine how to meet client needs and ensure needs to be referred are manageable and specific. 6. Maintain accurate and detailed client records. 7. Maintain accurate records of agency services and maintain referral relationships. 8. Maintain accurate records of church ministries and volunteer individual resource inventories. 9. Maintain referral relationships with Love INC community and partner churches, church ministries, and support Church Ministry Coordinators/Love INC Ministry Coordinators. 10. Produce Connection Center activity reports when needed. 11. Accurately enter Client information and progress into database software. 12. Keep accurate records of personal timesheet, vacation time, sick time, flextime and volunteer hours. 13. Along with the Connection Center Coordinator, the Case Manager will assist with training and supervising intake volunteers as needed. 14. Meet with clients for intensive, non-intensive and contact case management and work with transformational ministries to best meet the clients' needs. 15. Assist in developing Transformational Ministries in Eastern Sussex County. Gather Volunteers to teach Love INC courses Partner with Churches to host courses Enroll neighbors into courses Keep record of attendance and progress of neighbors enrolled in courses 16. Assist in developing and maintaining relationships with churches, volunteers, and partners. 17. Work to case manage individuals assisted in hotel program (or programs like it) as needed. 18. When needed assist the Director of Case Management in fulfilling the following when it comes to Love INC's transitional housing program: Create and maintain all applications, documentation, files, and data pertaining to Transitional Housing. Update as necessary. Implement and oversee all move-in and move-out/ termination procedures with qualifying neighbors. Assess and determine eligibility of neighbors for Transitional Housing based on qualifications and unit availability. Meet with and assess qualifying families in Transitional Housing weekly to ensure they continue to meet resource requirements and are progressing in their case goals. Perform weekly and bi/weekly property inspections and maintain all records. Communicate with appropriate teammates/ partners concerns regarding property issues and regular maintenance. Work with the Program Director to ensure all qualifying neighbors in Transitional Housing are actively participating in classes, mentorship or any other beneficial programming. Be on call for any emergencies pertaining to Transitional Housing and respond in a timely manner. Prepare documents, statistics, and agreements as needed for partnership, volunteer or donor meetings pertaining to Transitional Housing. Lead and/or participate in meetings as necessary. Correspond with Transitional Housing Partners as necessary via email, phone, zoom and in-person. Work with Community Relations Director to facilitate church ministry partnerships pertaining to Transitional Housing and maintain all correspondence. Work with Ministry and Community Relations Director to coordinate and oversee church ministry groups and individual volunteers participating in property improvement or administration tasks pertaining to Transitional Housing. 19. Assist in Managing the Connection Center in the absence of manager and coordinator. 20. Flexibility in working remotely at different Love INC sites on cases as determined by need. 21. Maintain and submit monthly statistics pertaining to case loads 22. Follow proper procedure when working with outside agencies on behalf of a neighbor by requesting and submitting Release of Information Forms prior to case discussion. 23. Be available to participate in additional department or team events outside of normal business hours, including occasional evenings and weekends.
Qualifications:
1. Be adaptable to the constant changes of a growing and busy essential non-profit. 2. Basic accounting and following contract requirements. 3. Computer proficiency (including Microsoft Office Word and Excel) 4. Strong discernment skills with a non-judgmental, empathetic, and person-centered approach. 5. Healthy, professional boundaries. 6. Good telephone, internet, and communication skills. 7. Ability to work independently and cooperatively with the whole team. 8. Demonstrated reliability, strong time-management skills, and commitment to confidentiality. 9. Reliable transportation.
