Job Description
Responsibilities The Case Manager is responsible for individual, family and psycho-educational groups, multidisciplinary treatment planning, and appropriate discharge planning through contact with community resources and assessing the patients' environment. The Case Manager is responsible for patient care on the assigned unit as well as other units as needed within the facility. Qualifications Bachelors degree in Social Work or A minimum of four years of work experience in the mental health field . A minimum of two (2) years of work experience in a behavioral healthcare setting, preferably in a psychiatric facility.
