Job Description

Position Summary:
The Neighborhood Center of West Volusia (NHCWV) Prevention Services Case Manager will assist residents of Volusia County using agency-managed projects and programs that enable clients to prevent homelessness and associated issues. This role involves assessing client needs, developing personal financial plans, and connecting clients with resources to improve their financial health. The Case Manager has strong analytical skills, excellent communication abilities, and a passion for helping individuals achieve holistic wellness and prosperity.
Duties and Responsibilities:
Service Coordination Conduct comprehensive assessments of prospective clients' financial situations, including income, expenses, debts, and assets; current crisis that requires financial assistance; and actionable short-term and long-term financial goals. Verify the client's eligibility for, and funding availability, of various NHCWV grants, to determine which program best meets the clients' needs. Support prospective clients in creating customized Individual Service Plans (ISP) based on current needs and assets that emphasize budgeting, debt reduction, and money savings strategies. Recommend financial products and services as needed, while ensuring that any financial advice complies with relevant laws, regulations, and agency policies. Facilitate referrals to supportive programs and community resources. Case Management Regularly follow-up with clients to track progress toward financial goals and adjust plans as needed, up to one year after program entry. Ensure ongoing financial education by referring clients to workshops and events in the community, or facilitating one-on-one education meetings. Assist the Prevention Services department in facilitating new client intake assessments, explaining processes and procedures, and referring to internal agency services within other departments, as applicable. Engage in ongoing professional development about financial resources, tools, and methods to assist clients. Record-Keeping and Communication Maintain detailed and accurate records of client intake documents, client interactions, assessment interviews, and progress reports within the records database. Maintain client confidentiality in accordance with agency and funder policies. Prepare and communicate client outcomes and program effectiveness reports for the Prevention Collaborate with volunteers and employees to ensure cohesive delivery of services and client care. Departmental Collaboration Support Prevention Services department operations by assisting in planning and facilitating special events, including, but not limited to: bi-monthly Mobile Food Drop grocery giveaways; Summer, Thanksgiving, and Christmas food distribution; and winter holiday toy giveaways. Receive cross-training to facilitate food distribution through the Choice Pantry in accordance with food safety and partner agency standards. Alongside department case management team members, perform data entry for non-financial services into the respective database, including, but not limited to: food, healthcare referrals, childcare vouchers, and vouchers to NHCWV Thrift Store.
Qualifications, Experience, and Abilities:
Education:
o Associate Degree (or above) in Social Work or related field or (2) years of experience working with the homeless population, preferred. o High School Diploma or GED equivalent required.
Experience:
o Minimum two years' experience in low-income property management, working with families or clients on an individual basis for teaching, counseling, and mentoring, preferred. Specialized knowledge, skills, and abilities: o Proficiency with Microsoft Office applications (i.e., Word, Excel, and Power Point) and HMIS. Sound decision making with solutions-oriented focus. Prompt and dependable, with the ability to work a flexible schedule as needed. Time management and the ability to multi-task. Strong organizational, analytical, and oral/written communication skills with the ability to cooperate on a team.
