Job Description

The Family Service Facilitator is responsible for overseeing the authorization and utilization of services. This position within the Utilization Management department is intended to ensure the maximization of resources in authorizing services for clients within our system of care. This position reports to the Director of Utilization Management or designee.
ESSENTIAL JOB FUNCTIONS
Oversees the authorization and utilization of services to ensure resources are maximized. Authorizes service allocation and coordinates service delivery, including crisis intervention linkages, with providers and community resources. Ensures family support and placement stabilization through linkage and consultation. Consults with Child Welfare staff inquiring about connecting families with existing resources within the structure of the Florida Safety Decision Making Methodology. Participates in meetings/staffings and coordinates activities between various stakeholders within the System of Care. Reaches out to existing community resources to make connections that will benefit families served. Fosters relationships with care managers, courts, education system and service providers. Participates in utilization review, tracking, and management of services to ensure the appropriateness of level of care, service provision, and expenditures. Displays understanding of, and sensitivity to, service population's cultural and socioeconomic characteristics. Performs other related job duties as assigned.
Education and Experience:
Bachelor's degree in Counseling, Social Work, or related field or equivalent; Master's degree in the like preferred; and two (2) years of relevant experience in the areas of child welfare, mental health, and/or utilization management; or an equivalent combination of education and experience.
Special Qualifications:
Certified as a Child Protection Professional (CPP) or CPP eligible preferred. Possession of a valid, State of Florida driver's license to operate the motor vehicle operated. Requirement exists at the time of hire and as a condition of continued employment.
