What You'll Do As a Discharge Planner, you will play a key role in supporting patient care and recovery by: Establishing and maintaining a welcoming, therapeutic relationship with clients Collaborating with physicians, nurses, emergency screeners, counselors, and other staff to ensure high-quality patient care Coordinating discharge planning and follow-up care to reduce readmissions Conducting family meetings and gathering collateral information to support treatment and discharge plans Actively participating in daily treatment team meetings to assess client progress and outcomes Leading psycho-educational groups and therapeutic activities as directed Assisting in client safety interventions, including participation in restraints when necessary Supporting program and departmental goals through additional duties as assigned
Who We're Looking For Minimum Qualifications:
