Job Description

Schedule/Hours:
Wed-Sun 2pm-10pm This position is responsible for: The Emergency Beds Program Case Worker is responsible for the intake process, bed assignment, orientation, case management, and daily operations of the emergency overnight beds program. This position includes assessing client needs, coordinating intake, managing bed assignments, providing case management services, and ensuring the program runs smoothly. The Emergency Beds program case worker will also collaborate with the Jacksonville Fire Rescue Department (JFRD) and work directly with individuals in crisis to meet their immediate needs while connecting them to long-term housing and support services
Key Responsibilities:
Manage all referral contacts received from JFRD. Establish eligibility for services, including possible transition to the Transitional Housing program. Provide counseling, support, and referrals to ensure client needs are met, including income, benefits, and psychosocial services. Maintain accurate and confidential client records, ensuring compliance with legal and ethical standards.
Physical Requirements and Working Conditions:
Works is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like. Bachelor's degree in social work, Behavioral Science, or a related field, and Three years of progressively responsible experience providing case management services, including client needs assessments and development of long-term action plans. Any equivalent combination of training and experience that provides the required knowledge, skills, and abilities.
Licenses and Certifications:
