Job Description

The Guardian Representative/Case Manager (part-time) is responsible for managing a case load of individuals under guardianship with responsibilities including but not limited to: assisting individuals with decisions regarding medical procedure recommendations and residential placement.
Responsibilities :
Visit ward's at least monthly. Document all activity performed on the ward's behalf in the ward management database within 10 days of activity being performed. Periodically assess the ward's functional capabilities. Assess and monitor care provision to wards in the areas of residential and social. Authorize expenditures for wards based upon maintaining health, safety, and well-being. Coordinate ward's medical care and mental and social services. Complete court reports in a timely manner. Liaison with court when needed regarding status of cases. Maintain caseload-related statistics.
Education and Experience Requirements :
Bachelor's degree in Social Work or related field. Must have at least 2 years of experience in case management or related experience. Must successfully undergo a Level 2 criminal background and credit history. Must have general understanding of human services and public benefits. Must enjoy working with older adults and persons with disabilities. Requires regional travel approximately a 100 mile radius from central office Strong verbal and written communication skills. Strong knowledge of Microsoft Office, including Word, Excel, Adobe Acrobat Pro and Outlook. Ability to learn new computer database management systems. Ability to work independently with minimal oversight, manage large workloads and keep deadlines. This position is part-time at 25 hours per week, with the schedule to be determined by the Manager and Employee. Paid holidays Paid time off
