Job Description Help for Job Description. Opens a new window. To recruit clients and provide program services and case management for clients participating in the Family Preservation & Economic Stabilization Program. This is an entry level position.
Responsibilities:
Identify, locate, and recruit farm workers in the community, conduct outreach activities and home visits Inform and educate clients about services available in the community. Complete client intake, family assessment and other required case management and record keeping activities and forms. Maintain complete, accurate and confidential client files including case notes. Input client data into the agency's database and CMIS, complete tracking and outcome reports measurement forms, and close client files according to the program guidelines. Monitor and work to meet program's goals as directed by supervisor. Provide transportation to farmworker clients to medical, social services & other appointments as needed. Provide language interpretation/translation, orientation and advocacy at appointments for clients. Assist clients with negotiating services (applying for programs, reading bills, filling out applications, etc). Provide referral information to clients and make referrals to other partners and agencies as needed. Facilitate support group and conduct financial literacy workshop. Attend staff meetings and represent the FWCC at interagency meetings and other mandatory training as requested by supervisor. Develop and maintain relationships with other community partners. Assist with translation, production and dissemination of flyers and printed materials. Pick up donations, supplies and provide donor receipts. Complete time sheets and all other agency paperwork in a timely and organized manner. Perform other duties as assigned by supervisor. Work as team member with other staff. Maintain a professional attitude.
Qualifications:
18 Years or older due to the nature of the job High School Diploma or equivalent, college degree in social work, human services, psychology or related area preferred. Must be bilingual in English/Haitian Creole, Spanish is a plus. Computer knowledge necessary. Must be service-minded with professional appearance and attitude. Driving record check
Physical Requirements:
Ability to drive safely and to carry out the above responsibilities. Must be able to bend, stretch, reach, grab, push, pull, hold and carry. Must be able to perform repetitive motion work. Lift up to 60 lbs occasionally.
