Your Specific Responsibilities:
- Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact.
- Provide Case Management to pregnant women and infants referred to the Healthy Start Program.
- Provide face to face contact with clients either in the OB provider's office or in the client's residence.
- Provide Parenting Education one on one with clients.
- Provide Breastfeeding Education and Tobacco Cessation Education to individual clients.
- Document all client contact information in the web-based computer application known as the Well Family System
- Provide referrals to community resources to supplement Healthy Start services.
Required Knowledge, Skills, and Abilities:
- Fluency in English and Spanish
- Experience working with pregnant women and children.
- Problem solving skills related to advancing client self-sufficiency.
- Ability to work well with others as a team.
- Computer skills: competency and experience.
- Experience in interviewing and assessing to determine the needs of the client.
- Knowledge of community resources.
Qualifications:
Minimum-- Valid Florida Driver's License
- Minimum of four-year college degree in one of the following areas; Social Sciences; a health-related field such as Nursing, Health Education, Health Planning, or Health Care Administration; or Social Work as required by FL Healthy Start Standards and Guidelines (Chapter 6-2)
- At least 1 year experience working with high-risk pregnant women and/or families
- Must have reliable and dependable transportation every day for home visitation purposes Preferred-
- 1 to 3 years of experience working with families
- 1 to 3 years home visiting experience
- 1 to 3 years case management experience
- Bilingual with fluency in
