POSITION OVERVIEW
Community-based case management role serving older adults and their families Provide both short-term and long-term case management services Develop individualized care plans and coordinate community resources Conduct home visits and maintain ongoing client communication Help seniors remain independent and safely age in place Strong autonomy with support from a dedicated team
COMPENSATION
Annual Salary:
$55,000 Fully Employer-Paid Health Insurance Retirement Plan with 6% Employer Match Starting Day One 20 PTO Days 12 Sick Days Life Insurance Coverage
SCHEDULE
Monday-Friday Schedule 9:00 AM - 5:00 PM No Weekends No Call Requirements Community-Based Work Environment ROLE Conduct client assessments and develop individualized care plans Coordinate healthcare, social services, and community resources Perform home visits and ongoing case management follow-up Advocate for older adults and their families Maintain documentation and care coordination records Support clients in maintaining independence and quality of life

Bachelor's Degree in Social Work
REQUIRED
Master's Degree in Social Work Preferred Case Management Experience
REQUIRED
Experience Working with Older Adults Preferred Comfortable Conducting Home Visits
REQUIRED
Strong Communication and Organizational Skills
REQUIRED
Ability to Develop Individualized Care Plans
REQUIRED
