Psychiatric Case Manager/Discharge Planner

Job Description

Job Overview We are seeking a dedicated and compassionate Psychiatric Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive mental health and behavioral health services for patients across inpatient, outpatient, and post-acute care settings. Your expertise will help facilitate patient recovery, discharge planning, and ongoing support, ensuring seamless integration of care within complex hospital environments. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working within a fast-paced, multidisciplinary team committed to excellence in mental health care. Responsibilities Conduct thorough patient assessments to determine mental health needs, including diagnostic evaluations and behavioral health screenings. Develop individualized care plans in collaboration with physicians, social workers, and other healthcare professionals. Manage utilization review and discharge planning processes to optimize patient outcomes and resource use. Coordinate services across inpatient units such as
ICU, PICU
(Pediatric Intensive Care Unit), and emergency departments, ensuring timely intervention and appropriate follow-up. Maintain accurate medical records, documentation of patient interactions, and compliance with HIPAA regulations. Facilitate intake procedures for new patients, including mental health counseling and addiction counseling assessments. Support post-acute care transitions by liaising with outpatient clinics, managed care organizations, and community resources to ensure continuity of care. Qualifications Proven experience in hospital settings with exposure to Level I Trauma Centers or Level II trauma centers is highly preferred. Strong background in acute care environments such as ICU, emergency medicine, or behavioral health units. Knowledge of medical terminology, physiology, and diagnostic evaluation techniques relevant to mental health cases. Experience with inpatient and outpatient case management, discharge planning, utilization management, and medical records documentation. Familiarity with social work practices, addiction counseling, and patient service coordination. Ability to perform comprehensive patient assessments within fast-paced hospital environments including pediatrics (PICU) when applicable.
