As a Case manager, you will facilitate our clients' care by assessing their needs, evaluate treatment options, plans, and be involve in evaluating various services to help our clients gain function and independence. Case Manager will help clients connect with resources that can help them with their challenges, whether they be mental health or substance abuse. The ideal candidate will have knowledge of mental health disorders, treatments, and community resources. Bilingual preferred but not required, Spanish/Creole.
