- Full-time Provide case management services for children ages 11
- 21 with mental health and/or co-occurring disorders as part of the Children's Community Action Treatment (CAT) Team in Charlotte County.
Provide services in the community; including school, home, etc. Interact with community partners and other stakeholders, as appropriate. Hours include evenings and on-call, as necessary or assigned. Conduct assessments, create treatment plans, coordinate and provide treatment and supportive services to children and their families. Provide appropriate referrals for additional services and resources, as identified. Provide support and advocate for children and family's rights and needs.
