Job Description
Position Overview:
The Ready4Release Case Manager meets with currently incarcerated inmates (jail or prison) to provide an overview of the Ready4Work program. When possible, the Ready4Release Case Manager will also complete a Plan of Care consisting of short-term goals to get the inmate ready for re-entry into the community.Reports to:
Site Manager Essential Duties and Responsibilities:
Travels to Florida Department of Corrections Prisons (travel up to 3 hours one way in a company vehicle) to promote the Ready4Work Program. Promote the Ready4Work program inside county jails throughout the State of Florida, and with other community partners. Facilitate InsideOut Dad® evidenced based-education programming to incarcerated fathers. Training on InsideOut Dad® programming will be provided. Complete a Plan of Care with short term goals for currently incarcerated individuals. Inputs plan of care goals and notes into the client record/database software system. Provides follow-up visits and supportive sessions to the inmates either face to face or via electronic communication. Responds to letters received from Inmates that are interested in the program. Participate in multidisciplinary staffing meetings as part of the Clinical Team which consists of Mental Health, Resources, Family Support Specialists, Career Development and Employment Services. Attend weekly and monthly meetings. All other duties as assigned.Education / Experience
/Qualifications:
Bachelor's degree required (Criminal Justice, Social Work, Sociology, or related fields). Master's degree preferred. In lieu of degree, 6+ years of relevant experience as a Case Manager. At least 3 years' experience working with marginalized or underserved populations. At least 1 year of experience working with clients with mental health and/or substance abuse disorders. High integrity and behaves ethically, conducting oneself with the utmost honesty and transparency. Excellent verbal and written communication skills. Proficiency in using computers, running reports, and learning new software. Proficiency in Microsoft Office Suite; Word, Excel, PowerPoint, Outlook, Teams, and electronic client records. Typical hours are Monday through Friday 8:30am-4:30pm; however, sometimes the hours extend to 7:00 AM - 6:00 PM. Must have a valid driver's license, clean driving record, and reliable transportation. Must be able to clear a Level II Background check through the Department of Corrections. Must be able to lift up to 25 lbs.Job Type:
