Elder Care Services currently serves as the Lead Non-Profit Agency for in-home services in Leon and the surrounding counties. Our comprehensive services provide community based, compassionate care for a broad spectrum of seniors. For active seniors, we provide an avenue to stay engaged in the community through volunteering placement. For families beginning the aging process, we are an advocate and trusted resource. For frail and homebound seniors, we continue to provide a variety of services that promote independence and safety while individuals age in place. We are looking for a helpful, compassionate, and highly motivated Case Manager to join our team . As our Case Manager, you will have the chance to improve the lives of seniors in the Big Bend We will provide an hourly rate starting at $17.50 per hour and a full benefits package (see below for details). We are specifically looking for a self-starter who enjoys staying busy, works well in a fast-paced and high volume atmosphere, and has a passion for service. If this describes you, please read on! Qualifications and Education Requirements A bachelor's degree in social work, sociology, psychology, nursing, gerontology, or a related social services field is required; four years of related job experience or any combination of education and related experience may be substituted for a bachelor's degree upon approval of the Area Agency on Aging. Basic computer skills and knowledge required. Reliable transportation and a good driving record is a must, as travel within an 80 mile radius is required. Successful completion of Area Agency on Aging for North Florida Case Manager certification as made available by the Area Agency on Aging required. Successful completion of Area Agency on Aging 701B Assessment training within 10 business days of hire required. Availability to work beyond the hours of the usual workday from 8:00am-5:00pm, when necessary, required. Full wrist dexterity at a repetitive motion and the ability to stoop, bend, and lift up to 25 pounds required. While performing the duties of this position, the employee will spend a majority of time in client's homes and will be frequently exposed to a variety of client conditions and elements. The noise level is moderate to quiet. Duties and Responsibilities 1. Process referrals and complete initial client assessments in order to identify: Problems and immediate needs of the client Client's strengths and ability to cope with his/her limitations Caregivers and/or agencies providing assistance Gaps requiring intervention 2. Assist client enrollment in appropriate program including: Educating client about programs available and qualifications Assist client in completing appropriate application/paperwork for enrollment Assure client enrollment requirements are met 3. Provide information and counseling as needed to client and caregiver. 4. Involve the client in the care plan development. 5. Develop care plan and assure care plan approval is obtained. 6. Arrange, coordinate, and monitor client services outlined in care plans either through ECS or outsourced provider. 7. Review care plans with other professionals involved with service provision to the client. (Outsource providers, senior companions, etc.). 8. Provide follow-up (as per program requirement) to assure client satisfaction. 9. Complete client visit and contact as per program requirements and time frames. 10. Review the service delivery impact on the client as per program requirement but at a minimum of every six months. 11. Complete a written client reassessment annually (within 365 days). 12. Complete annual paperwork in timely manner (within 365 days). 13. Complete case notes and assure billing of case management services in timely manner. 14. Maintain caseload/unit production, program standards and goals. 15. Assist clients in obtaining equipment and supplies (including ordering supplies in a timely manner). 16. Assist fiscal in requesting client payment of co-pays or advocating for co-pay adjustments as needed. 17. Advocate for the needs of clients and caregivers. 18. Maintain confidentiality and security of Protected Health Information (PHI) according to HIPAA guidelines. 19. Receive and respond to referrals from CARES and Adult Protective Services in required time constraints. 20. Perform other duties as assigned by the Clinical Director and/or the President/CEO.
