Position Summary:
The Outreach Case Manager is a boots-on-the-ground, field-based professional responsible for engaging individuals and families experiencing homelessness, including veterans, in streets, encampments, shelters, and community settings. This role combines intensive outreach, comprehensive case management, and benefits navigation expertise to connect clients to housing, healthcare, income support, and stabilization services. The Outreach Case Manager must have skilled knowledge of mainstream and veteran-specific benefits, as well as demonstrated experience with HMIS documentation, data entry, and reporting requirements.Required Qualifications:
Bachelors degree in social work, Human Services, Psychology, or related field (or equivalent experience) with minimum of 2 years of experience working with individuals experiencing homelessness. And/or High School diploma with equivalent background experience with four years of experience working on the same setting. Demonstrated experience conducting street outreach. Skilled working knowledge of mainstream public benefits and veteran-specific resources. Ability to learn data entry systems and compliance knowledge. Valid drivers license and transportation. Ability to work flexible hours, including early mornings or evenings as needed.Essential Duties and Key Responsibilities Street & Community Outreach:
Conduct proactive outreach in encampments, shelters, hospitals, libraries, and other community locations. Build trust and rapport with unsheltered individuals using trauma-informed and person-centered approaches. Complete vulnerability assessments (e.g., VI-SPDAT or similar tools) and coordinated entry documentation. Respond to referrals from community partners, law enforcement, hospitals, and service providers. Participate in case conferencing and coordinated entry meetings.Case Management & Housing Navigation:
Conduct comprehensive biopsychosocial assessments and develop individualized service plans. Assist clients in securing emergency, transitional, and permanent housing (RRH, PSH, HUD-VASH, etc.). Support housing stabilization, landlord mediation, and retention planning. Coordinate referrals to mental health, substance use treatment, medical care, employment, and supportive services. Maintain consistent follow-up and field visits to ensure service engagement.Veteran Services & Benefits Navigation:
Identify and verify veteran status. Assist eligible clients with enrollment and navigation of services through the U.S. Department of Veterans Affairs (VA). Support applications for VA healthcare, disability compensation, pension benefits, andHUD-VASH.
Coordinate with local VA Medical Centers, SSVF providers, and veteran service officers. Advocate for expedited services for vulnerable veterans.Mainstream Benefits & Income Supports:
Assist clients with applications and renewals forSSI/SSDI
(including SOAR-informed practices). Support enrollment in SNAP, Medicaid, TANF, General Relief, and other public benefits. Facilitate acquisition of identification documents (birth certificates, Social Security cards, state IDs). Provide budgeting guidance and financial literacy support.HMIS & Documentation:
Enter timely, accurate data into HMIS in compliance with HUD and local Continuum of Care standards. Maintain complete, audit-ready client files (electronic and/or paper). Ensure data quality, confidentiality, and adherence to agency policies. Generate reports and documentation for program compliance and performance outcomes.Collaboration & Community Engagement:
Maintain consistent working relationships with shelters, housing providers, landlords, VA partners, and community agencies. Participate in case review meetings and interdisciplinary team discussions. Represent the agency at community meetings and outreach coalitions.Core Competencies:
