Case Manager - Bacon CS

Job Description:

Performs assessments, case management, and treatment planning/monitoring for day and residential clients. Performs, coordinates, and monitors the assessment, treatment planning process, and follow up for each client. Maintain day service and residential records. Advocate for clients to receive needed medical care/DFACS appointments and follow up. Provides social support needed to cope with various social situations.

Job Responsibilities:

Will work Monday - Friday, 8 AM - 4 PM.

Competencies:

Knowledge of diagnostic impressions or criteria. Knowledge of how client advocacy system operates. Knowledge of individual and group therapy techniques. Knowledge of various therapeutic models and intervention as directly related to area of assignment. Knowledge of crisis intervention techniques.

Required Minimum Qualifications:

Must be 21 years old to apply. Bachelor's degree in social services related field. One-year Intellectual/Developmental Disabilities experience preferred. DDP eligible. Experience with or knowledge of trauma informed, resilience-oriented care preferred.
