In home assessments are required for this position within 1 hour drive time of home location
Job Summary:
Under direction, the Social Worker/Case Manager performs work of moderate difficulty by providing skilled casework services to selected caseloads or clients with special problems such as health disability or those at risk of nursing home placement; provides specialized casework services aimed at securing the client's overall well-being and maximum degree of independent functioning. Serves large geographic areas which may include one large county and/or many small counties which may involve extensive travel, and performs related work as required.
Duties and Responsibilities:
Completes all assessments, task lists, and care plans as identified by services requested. Completes all documentation within established Standards of Promptness. Ensures that the patients and caregivers understand the patient's rights and responsibilities. Utilizes time management techniques when scheduling patients and planning daily work activities.
Qualifications:
Bachelor's degree in social work, sociology, psychology, or a related field Two years' experience in the human service or health related field. Experience in
EDWP/CCSP
waiver program preferred. Leadership experience preferred. Ability to effectively coordinate and communicate with clients, service providers, general public, and other staff members. Reliable transportation with current driver's license and proof of liability insurance. Satisfactorily complete a criminal background check. Participate in agency and program orientation, and annual continuing education as required by program.
