4/3/2026 About the Role Clarvida is seeking an ACT (Assertive Community Treatment) Case Manager to support adults experiencing severe mental health challenges. In this role, you will partner with clients to develop and implement personalized service plans, focusing on areas like housing, employment, therapy, medical care, and substance use support. You will work directly with clients in their communities to improve their quality of life and facilitate their recovery journey. As an ACT Case Manager, you will collaborate with a dedicated team, provide hands-on support for daily living skills, and connect clients to necessary resources. You will also help track progress through home visits, offer transportation, and contribute to team-based client goals.
