Job Description
We are hiring a Case manager for our Developmental Disabilities program! This position is Monday - Friday 8am- 5pm. The Case Manager also known as the Developmental Disability Professional (DDP) maintains a caseload of Intellectual Developmentally Disabled individuals. The DDP is an advocate for the individual's wants and needs. The DDP is responsible for advocating at the individuals at ISP meetings, SIS meetings, etc. The DDP is a liaison for family members, support coordination, host home providers and the community.
