Job Description

Job Summary We are seeking a dedicated and compassionate Case Worker to join our team. The ideal candidate will be responsible for providing one-on-one services to youth and families involved with DFCS and in the juvenile court system. These services would include counseling (upper level) or tutoring, mentoring, anger management, conflict resolution, referrals to services, and case management (Bachelor's level). The Case Worker will work closely with BTC staff and the juvenile court/DFCS to provide comprehensive support for clients. Duties Conduct one-on-one sessions with youth and families. Coordinate services for families as needed and refer to resources. Work closely with the juvenile court and DFCS to inform Case Managers of all updates, issues or needs of clients. Teach anger management skills, conflict resolution skills, and tutoring services. Complete a case note for each session conducted detailing interventions. Collaborate with all agencies involved with each youth/family. Monitor client progress and adjust case plans as necessary based on ongoing needs. Advocate for clients' needs to ensure they receive appropriate services. Requirements Previous experience in case management within social services is preferred. Familiarity with working with at-risk youth and families. Knowledge of HIPAA regulations and confidentiality. Experience in case documentation. Understanding of addiction principles, juvenile law, and DFCS cases is desirable. Excellent communication and interpersonal skills to effectively interact with clients, families, and team members. Ability to work collaboratively within a multidisciplinary team environment. Strong organizational skills and attention to detail.
